What do you do in the pre-orientation stage of building therapeutic relationships?
Self assessment/self-awareness. Reviewing existing patient data. Reflect on own experiences, values, beliefs, and feelings as they relate to the patient. What are my anxieties, fears, and other feelings. Self-consciousness, sometimes self-doubt. Review patient needs, learn about his situation.
What do you need to do in the beginning phase of the therapeutic relationship?
Goal: develop trust and rapport. Activities: introduction, nonthreatening conversation, discover commonalities, convey empathy, interest, caring, respect. Engender Goodwell. Establish a contract
What do you do in the middle stage of the therapeutic relationship?
Be certain you have a stab Lish trust and rapport before engaging in the following activities. Complete assessment. Jointly establish and affirm goal, it’s a spacer following: resistance, transference, countertransference
What is resistance?
To resist further work in progress to control anxiety
What is transference
Emotional responses by the patients toward the nurse, either positive or negative. An unconscious response where in the patient relates the nurse in a manner akin to earlier relationship. Patient transfer the emotions and or experiences in the earlier context to the present situation
What is countertransference?
It is similar to transference, but experienced by the nurse and directed towards the patient. The nurse can experience emotional reactions with the patient. Can’t be positive or negative emotion
If the patient exhibited resistant, how should the nurse respond?
Slow down the process. Discuss your observation (use immediacy). Anxiety reduction intervention. Review or revise the contract (change goals)
What is an impasse?
A situation in which further discussion of a topic or any further interaction on any topic, is blocked or no longer possible
What are common causes of impasses?
Patient resistant (usually due to anxiety that the topic). Nurse most prematurely into working days before track and rapport are sufficiently established
What is the termination process of the therapeutic relationship
Concluding the relationship therapeutically. Preparing for the patient for an eventual end of the relationship actually begins during the beginning phase. Nurse notes the patient movement through the therapeutic process, noting and reinforcing the patients process. Nurse prepares patient for what to expect and how to respond, how to care for self after discharge. Nurse know it’s common reactions to the impending end of treatment, discuss his feelings, assist the patient to anticipate and adjust to these: sense of loss, grief, return of resistance, regression
What is cultural care preservation
Cultural believes are not a barrier to treatment and are preserved
What is cultural care accommodation
Culture conflict with the care to a degree; negotiation and changes to care resolve the conflict
Cultural care repatterning
Culture conflict fears with treatment; nurse reframe cultural believes and guys the family to alter their cultural perspective so as to be compatible with treatment
What is anosognosia
Lack of insight/lack of awareness
What are the stages of therapeutic relationships?
Pre-orientation (preparation), beginning (introductory or orientation) middle (working), resolution (termination)
