Digoxin (Lanoxin)
Therapeutic 0.5-2 ng/mL
Toxic > 2.2 ng/mL
Digoxin toxicity—confusion, loss of appetite, nausea, vomiting, diarrhea, change in vision such as blurry or yellow vision, fatigue, fast or irregular heartbeat. Slow heartbeat—dizziness, feeling faint or lightheaded, confusion, trouble breathing, unusual weakness or fatigue.
Lithium (carbolith)
Therapeutic: 0.5-1.2 mEq/L
Toxic: > 1.5 mEq/L
Valporic Acid (Depakote)
Therapeutic range: 50-125 mcg/mL
Toxic range: >125 mcg/mL
Carbamazine (Tegretol)
Therapeutic range: 8-12 mcg/mL
Toxic range: > 12 mcg/mL
Phenytoin (Dilantin)
Therapeutic range: 10-20 mcg/mL
Toxic range: > 30 mcg/mL
Theophylline
Therapeutic range: 10-20 mcg/mL

Lidocaine
