therapeutic contact lenses Flashcards
According to the CLEAR report, what can therapeutic contact lenses be divided into?
Bandage CLs and Rehabilitative CLs
What do bandage CLs do?
Treatment of ocular discomfort or support/ protect the cornea
What do rehabilitative CLs do?
Prescribed when px struggling to achieve good visual becasue of RX or cosmetic result after trauma
What are the four types of therapeutic CLs?
- bandage
- lenses for chronic conditions
- light reducing
- cosmetic
What are some uses for bandage cls? There are loads
- Cross linking
- Persistent corneal epithelial defects
- corneal abrasions
- penetrating keratoplasty
- post pterygium surgery
- post-trabeculectomy bleb leaks
- corneal perforations and trauma
- entropion
What are the main similarities with the uses of bandage cls?
Issues with corneal epithelium and require protection
Do px who have had lasik or lasek require bandage cls?
No- there is no benefit to comfort no healing rate
Name one type of bandage cls?
Amniotic lenses
What is an amniotic lens produced from?
The amniotic sac of pregnant women
What is an amniotic cls used for? And why?
Recurrent corneal erosion, allows epithelial cells to regenerate
How is an amniotic lens put on the eye ?
Either with a soft cls on top or it is sutured on
How is a sutured amniotic lens removed?
It dissolves
Name four chronic conditions which therapeutic cls may be used in
- graft vs host syndrome
- Sjögren’s syndrome (linked to dry eye)
- Steven’s Johnson’s syndrome
- filamentary keratitis
What contact lenses are beneficial for px with chronic condition? And why?
Mini-scleral lenses,
corneal surface is protected and prevents further corneal insult + fluid layer keeps cornea hydrated
Can a px wear min-scleral lenses over night?
Yes in severe cases to stabilise epithelium
What are two indicators to use prosthetic lenses ?
1) improve cosmesis
2) reduce light entering the eye
In what condition would you want to reduce light entering the eye?
Albinism
What are the three ways cosmetic lenses can be produced?
- tinted
- screen tinted (looks like an iris)
- hand painted(hand painted to look like px iris)
What is a tinted lens?
Solid colours + no iris detail +come in a range of tint densities
How do you decide whether they have a clear or black pupil?
What they actually required the lens for; is it for cosmesis or light reducing
What does a screen printed lens allow for compared to a tinted lens?
Iris detail
What is a pro of screen printed cls?
Can be mass produces in a reproducible way
What is required for a success of a hand painted cls?
A clear photo taken of a px eye in good day light for colours to be matched
Do the cured and uncured colours match of a Hand painted cls?
No - the skill is the painter need to known what colour will form after the cl has been formed
Why would you give a px with strabismus a cls?
If they have a constant strab then for cosmetic purposes so their eyes look straight
What is involved in the initial fitting of a Therapeutic cls for strab pxs?
DATACT lens is put on px eye and a picture is taken of where these dots on the cls lie + another Pic is taken off other eye
After am initial fitting what is done with the two pictures taken?
It is sent to the lab and the artist will paint a Decentred iris so that when is put onto the px their eyes will look straight
What is another pro apart from cosmesis for a pc with Therapeutic cls for strab?
They will have no vision out of that eye _ no visual disturbances + they will have more confidence
What are special effect cls?
Used for characters in movies
