What does HAMS stand for
Health status (conditions)
Allergies to meds
med history (Rx, OTCs)
Social history (lifestyle)
What is the most common solution to drug therapy problems
Medication needed
What criteria does a patient not qualify as a self-care candidate
- has any severe symptoms
- any symptoms that persist or return repeatedly
- is self-treating to avoid medical care
What is the assess step
ANALYZE information that has been collected to identify any real or potential drug therapy problem
