A

Let them know you’re only able to complete the notarial certificate, all other portions of the document must be completed by them. They can call the issuing or receiving agency for more information on how to complete the form.

A

Report the incident to the police and then notify the Oregon Secretary of State’s using the Missing Stamp, Certificate or Journal form within 10 days.

A

Hand the document back to the client to have them fill it in. If the client doesn’t have the information, suggest they call the issuing or receiving agency for information on how to complete the form, or have the client line and initial the space. Note the location of the blank space in the additional information section of the journal.
