What is the nursing process?
Cyclical and dynamic in nature, client centred, focused on problem solving and decision making
Goals are to prevent health problems and minimize or alleviate problems
What is the 5 components of the nursing process? (ADPIE)
- Assessment
- Diagnosis
- Planning
- Implementation
- Evaluation
Define assessment
Collecting data
Holistic assessment
Can be collected from family, group or community
Define data collection
What are the 2 kinds of data collected?
Subjective data: symptoms, info from the patients pov, ex. Pain dizziness, feeling worried (WHAT YOU HEAR)
Objective data: signs, data that can be detected, observed or measured, seen by examination ex. Pale, sweating, yellow skin (WHAT YOU SEE)
Define diagnosis
Look at the patient holistically and look at potential health problems
Not a medical diagnosis
Ex. Trouble breathing, anxiety and pain
Risk factors
Define planning
Setting priorities, establishing outcomes, writing a care plan
What are the 3 phases of planning nursing care?
- Initial care: admission
- Ongoing care: hospital stay
- Discharge stay: what they need for home
Define interventions
Actions that care givers do to alleviate patients problems and prevent a problem from occurring
What are 2 types of nursing interventions?
- Independent: does not require a physicians order, requires critical thinking and judgement
- Dependent: does require a physicians order
Define implementation
Performing procedures, continue to collect data, communication, documentation
Define evaluation
Have the successful outcome happend? Future care planning
