Characteristics of the nursing process
Systematic, dynamic, interpersonal, outcome oriented, universally applicable
How is the nursing process systematic?
Each activity is part of an ordered sequence and each activity depends on the accuracy of the activity that precedes it.
How is the nursing process dynamic?
There is much interaction and overlapping of the five steps. No single step is a one time phenomenon. Sometimes, all five steps occurs almost simultaneously. Each step flows into the next.
How is the nursing process interpersonal?
It ensures that nurses are patient centered and not task centered. Encourages nurses to work together. Encourages patients to participate in their own care and the nurse to help patient to use their strengths to meet their own health needs.
How is the nursing process outcome oriented?
Nurses and patients work together to identify goals or “outcomes” that are important to the pt and promote health. Outcomes are measurable.
How is the nursing process universally applicable?
Provides a framework for all nursing activities. Works with any pt in any setting. Can be used in any nursing situation.
The parts of a nursing diagnosis
Problem, etiology and defining characteristics
Format of a nursing diagnosis
Problem “related to” the etiology “as evidenced by” the defining characteristics.
I.e. “Impaired skin integrity related to mobility deficit as evidenced by sacral ulcer.”
