Describe the anatomy of the acetabulum?
- Deepened and protected by fibrocartilaginous labrum
- hyaline lining
- centre filled with fatty tissue (+synovial sheath)
- cavity directed obliquely antero-laterally and inferiority
What is the trabecular pattern of the hip?
Superior to inferior compression trabeculae
Inferiolateral to medial tension trabeculae
-lead to osteoporosis + fracture of femoral neck
What 3 bursa can be found around the hip?
Greater trochanteric bursa
Ischial bursa
Iliopectinal bursa
What is angle of inclination?
What is more than normal?
What do we call these angles?
The angel from shaft of femur to neck of femur
= 125* aprox
> 125* = Coxa valgus
< 125* Coxa varus
What is angle of anteversion?
Formed by projection of long axis of femoral he and and transverse axis of femoral condyle
-should be 12*
Variations can cause toeing in / toeing out
Open/closed packed position of the hip?
Closed:
-full extension + internal rotation/abduction
Open:
- 10-30* flexion
- 10-30* abduction
- 10-30* External rotation
What is the capsule pattern of the hip?
Decreasing stiffness Internal rotation Abduction Flexion Extension Adduction External rotation
What is a trendelenburg sign?
Weakness of the Gluteus Medius = unsupported hip on opposite stride (walk like a model)
Trendelenburg sign: one legged standing stability
-pelvic drop on opposite side
What is the boarder and content of femoral triangle ?
Superior = inguinal ligament Medial = adductor longus Lateral = Sartorius
Femoral artery, nerve and vein
Trochanteric bursitis?
Cause, presents, pain?
Cause: overuse or direct injury
Present: pain felt primarily over the lateral hip region/greater trochanter
Pain: deep and aching
Bony palpation of hip?
Greater trochanters
Anterior, inferiorly, Posteriorly
Soft tissue palpation?
-Femoral triangle: inguinal lig, adductor longus, Sartorius
- femoral artery
- lymph nodes
- hip flexors, adductors, abductors
- sciatic nerve
- Ischial tuberosity
- hamstrings
- trochanteric bursa
Rang of motion for hip?
flexion: 135* Extension: 30* Abduction: 45-50* Adduction: 20-30* Internal: 35* External: 45*
Joint play motions of the hip?
Long axis distraction S-I glide in flexion S-I glide in flexion + internal S-I glide in flexion + external Extension
