What type of deaths must be reported to the Coroner?
Suicide.
Cause unknown or no Doctor’s Certificate issued.
Unnatural or violent death.
Medical or surgical or dental operation or procedure.
Birth deaths or deaths that appear to be the result of pregnancy or giving birth.
Institutional deaths (psychiatric hospitals, children’s homes, prison or police custody etc).
Enactment requires holding an inquest (AFDA, CYP & Families act etc).

List the facts that need to be established at an inquest:
That a person has died. (POL265 deceased person cert)
The person’s identity. (POL265A deceased pers ID form)
When and where the person died. (POL47 or 47A)
The causes of the death. (pathologist report)
The circumstances of the death. (statements, job sheets)
Outline the functions of a coroner under the Coroners Act 2006:
The coroner enquires into the manner of death of any person in any case where the Act requires that the deaths be reported to him or her.
When an inquest is required?
Suicide
Institutional deaths
Any death in which the Coroner is not satisfied that the cause or circumstances of death have been clearly established.
