(2-5mg every 5 mins to max 20mg)
Pain Management: 0.1mg/kg IV/IO
Max single dose of 10mg

Ketamine
Carried: 500mg/5mL vial
Dose: 0.2mg/kg (max 20mg) Slow push
May repeat 1x
Glucagon
Carried: 1mg/mL
Dose: 0.1mg/kg (max 1mg)
May repeat 1x after 10 mins
Use if you can’t get IV access
Diltiazam
Carried: 100mg vial
Dose:
Afib/Aflutter : 20mg IV/IO,
1st -10mg over 2 mins
2nd -10mg slow as BP allows
**Can repeat x 1
Over 65: 10mg IV/IO
1st- 5mg over 2 mins
2nd- 5mg slow as BP allows
**May repeat 1 more time
**SVT: Consider administering if adenosine and vagal maneuvers do not work
Use same dosages as above
EPI ( 1:1000)
Anaphylaxis/Allergic reaction dose

Dose: 0.01mg/kg IM (max 0.3mg)
Repeat every 5-15 mins

Dose: 1mg rapid IV/IO / repeat 3-5 mins (max 3mg)
Peds (>1yr) : 0.02mg/kg rapid IV/IO push
Min single dose 0.1mg
Max single dose 1mg
Repeat every 3-5 mins
Max total dose 3mg
-Anticholinergic
-Stops the parasympathetic (acetylcholine reaction)(slows down) response
-Allows for increase in HR and blood pressure
Indications- Bradycardia and organophosphate poisoning
1st degree and 2nd degree, type 1
Contraindications
Tachycardia
Hypersensitivity
TXA
Carried: 1,000mg/ mL
>14 years: Administer TXA 2 grams slow IV/IO push (over at least 2 minutes) if shock index >1 (HR/SBP)
Peds <14 years: Administer TXA 30 mg/kg slow IV/IO push (over at least 2 minutes) for patients with shock (per age-based cutoff)
Zophran (Ondansetron)
Carried: 4mg/2mL
Administer ondansetron 0.15 mg/kg IV/PO/SL ODT
Max single dose 4mg
Max total dose 8mg
May repeat x 1 in 15 min
Do not give if age <3mos
Midazolam (Versed)
Behavioral
Carried: 10 mg/ 2mL
Administer midazolam 0.1 mg/kg IM/IN
Max initial dose 10 mg IM/IN
May repeat x 1 at half initial dose after 10 minutes to a max total dose of 15 mg IM/IN
or
Administer midazolam 0.05 mg/kg IV/IO
Max initial dose 5 mg IV/IO
May repeat x1 at half initial dose after 10 minutes to a max total dose of 7.5 mg IV/IO
For patients with agitation that interferes with necessary patient care
Administer midazolam 0.05 mg/kg IM/IV/IO (max dose 2.5 mg) May repeat x1 after 10 minutes to a max total dose of 5mg IV/IO
14 or > 65 years max initial and total doses are half
8 yrs: Contact Medical Direction for orders
Midazolam (Versed)
Seizure
Carried: 10mg/ 2 mL
Administer benzodiazepines.
If age >60, consider reducing dose by half
0.2 mg/kg IM
Max 5mg if <40kg
Max 10mg if >40kg
Repeat x 1 if seizure continues
If seizure onset after IV established, administer Midazolam 0.1 mg/kg IV
Max single dose 5 mg
Administer slowly over 2 minutes
Albuterol
Carried: 2.5mg/3mL SVN
Asthma: repeat as needed
ANA: SVN Max 3 doses
Dextrose 10%
Dose :1mL/kg IV/IO
Max 250mL

Hydroxocobalamin (cyano kit)
Dose: 70mg/kg
Max 5g dose
May repeat x1
Calcium Chloride
Brady: Dose 1g IVP
Wide complex Brady and history of renal failure, consider hyperkalemia
Cardiac arrest: 20mg/kg IV/IO (max 1g)
