Glaucoma Restrictions

A

Must be certified as an optometric glaucoma specialist
Must consult with an ophthalmologist within 30 days of diagnosis of glaucoma
Before prescribing a beta blocker must take a complete case history and determine if the patient has a physical exam in the last 180 days
If no physical exam or history of congestive heart failure, bradycardia, heart block, asthma, or COPD must refer patient to ophthalmologist
