Increased troponins
A
Indicates necrosis of heart tissue (up to two wks prior), takes 12h for levels to express, i.e early troponin testing is pointless.
Increased troponins = Myocardial infarction, but can also be any other heart necrosis, myo/pericarditis, etc.

ECG changes
A
Acute coronary syndrome i.e angina or MI - angina will have no T wave or ST changes and no troponin change
ST elevation, 2mm in chest or 1mm in limb = STEMI etc.

Troponin and ECG normal
A
Endoscopy to confirm oesophagitis or spasm
Chest Xray/angiogram to check PI, pneumothorax, aneurism etc.

Mechanical pain, esp. if responding well to simple analgesia
