Lower abdo pain Swollen, painful testis Nausea Vomiting Increased HR High-rising/horizontal lying testis Thickened spermatic cord (early sign) Negative Prehn's sign (elevating testis does not relieve pain)

A
Urgent senior review Confirm diagnosis on USS as long as treatment not delayed NBM Analgesia (IV morphine with cyclizine) IV access Fluids

Infertility
Colateral torsion if not fixed
