Testicular torsion
A
Twisting of the spermatic cord with rotation of the testicle
Urological emergency
Typically teenage boy

Features
A
Severe, sudden onset pain
Pain may be referred to lower abdomen
Nausea and vomiting

On examination
A
Swollen, tender testis retracted upwards
Reddened skin
Cremasteric reflex is lost
Elevation of testis does not ease the pain (Prehn’s sign)

Management
A
NBM
Analgesia
Urgent senior urology assessment
Surgical exploration of the scrotum
Orchiopexy (correct position of testis and fix in place)
Orchidectomy if surgery delayed or there is necrosis

A
US (whirlpool sign)
Not recommended if it will delay patient going to theatre
