What equipment do you need?
Non-sterile gloves
Pen torch (cleaned)
(swabs if needed)
What are you looking for on inspection of the scrotum?
Erythema Bruising Necrotic tissue Ulcers Rashes Scars Swelling
Which testicle do you examine first?
The ‘normal’ one
How do you palpate the testicle and epididymis?
Use box technique to fix testicle in place
Assess size, shape, consistency, tenderness
How do you palpate the spermatic cord?
Roll it between thumb and index finger
What do you use the pen torch for?
Shine through scrotal sac
Solid masses will not allow light to shine through
What else do you palpate for?
Enlarged inguinal / supraclavicular lymph nodes
Inguinal hernia
What signs and reflexes should you check for?
Cremasteric reflex - stroke inside of thigh and observe scrotal skin contractions
Prehn’s sign - testicular pain relieved by elevating testes (epididymitis)
What rationale is there for carrying out a scrotal examination?
Pain Dragging sensation Trauma Urethral discharge Lumps / masses Scrotal asymmetry / swelling Routine screen
How does epididymis-orchitis present?
Dysuria, urgency, frequency Tender tail and head of epididymus Erythema and mild scrotal cellulitis Normal cremasteric reflex Positive Prehn's sign
How does an epididymal cyst present?
Lump (can be bilateral)
Well-defined, fluctuant and will transilluminate
