Follow format laid out by AGENCY – there is not a consistent format used in all settings

Becomes part of individual’s OFFICAL CHART ; often shared with other members of the team

Culmination of all your assessment efforts; represents YOU, YOUR DEPARTMENT AND TR PROFESSION

Professionally written, using appropriate language; written in 3rd person
Summarizes factual information; includes some additional interpretation and recommendations at end
Avoid too much T.R. “jargon”

Typed, single-spaced; generally 2-3 pages maximum
