Adverse drug event (ADE)
Any undesirable occurrence related to administration of or failure to administer a prescribed medication.
Adverse drug reactions (ADRs)
Unexpected, unintended, or excessive responses to medication given at therapeutic dosages (as opposed to overdose); one type of adverse drug event.
Allergic reaction
An immunological hypersensitivity reaction resulting from an unusual sensitivity of a patient to a particular medication; a type of adverse drug event and a subtype of adverse drug reactions.
Idiosyncratic reaction
Any abnormal and unexpected response to a medication, other than an allergic reaction, that is peculiar to an individual patient.
Malpractice
Improper or unethical conduct or unreasonable lack of skill that results in harm and where compensation may be sought. All malpractice involves negligence.
Medical errors
A broad term used to refer to any errors at any point of patient care that cause or have the potential to cause a patient harm.
Medication errors (MEs)
Any preventable adverse drug events involving inappropriate medication use by a patient or health care provider; may or may not cause the patient harm.
Medication reconciliation
A procedure implemented by health care providers to maintain/collect an accurate and up-to-date list of medications for all patients between all phases of health care delivery. (p. 99)
Negligence
Unintentional harm that results from conduct that does not meet a standard of care established by law.
Near miss/close call
Where an event could have resulted in unwanted consequences but did not

where an incident occurs but results in no injury to the patient,

An incident that causes harm
What is a critical incident
An incident that results in serious harm
