What is a failure?
Inability of a system or component to fulfil its operational requirements / perform its intended function for a specified time under specified environmental conditions.
What is error?
A design flow or deviation from a desired or intended state.
It is a static condition, a state which persists until removed.
May lead to a failure.
What is a fault
Hardware or software defect which resides temporarily or permanently in the system
What is accident
Undesired and unplanned but not necessarily unexpected event that results in at least a specified level of loss

What is an incident / near-miss
An event which involves no or only minor loss but with the potential for loss under different circumstances
What is a hazard
A state of the system (condition) that MAY lead to an accident
A situation where there’s actual or potential danger to people or the environment
