Tendon degeneration eg. Achilles tendon
-intrasubstance mucoid degeneration
–may be swollen, painful, tender; may be asymptomatic
–precursor to rupture?
Inflammation of Tendon (de Quervain’s stenosing tenovaginitis)
-tendons of EPB + APL passing through common tendon sheath at radial aspect of wrist
–swollen, tender, hot, red
–positive Finklestein’s test
Enthesiopathy
Inflammation at insertion to bone of:
- muscle/tendon
- ligament
Traction apophysitis - e.g. Osgood Schlatter’s disease
- insertion of patellar tendon into anterior tibial tuberosity
- adolescent active boys
- Recurrent load
- Inflammation
Osgood Schlatter’s disease symptoms
- pain at the tibial tubercle
- localised tenderness

-plain X-rays

Early stage
-modification of activities and non pharmacological measures
-non-steroidal anti-inflammatory drugs (NSAID)
-physiotherapy
Progressive or late stage
-surgery
Mallet finger
-insertion of extensor tendon into dorsum of base of distal phalanx of finger
–forced flexion of extended finger
Treatment Avulsion
Conservative –Limited application –Retraction tendon Operative -Reattachment tendon -Through bone -Fixation bone fragment
Intrasubstance Rupture e.g. Achilles tendon
-Load exceeds failure strength
-positive Simmond’s (squeeze) test
–palpable tender gap
Laceration (incision) e.g. esp. finger flexors (FDS & FDP)
- common
- males > females
- young adults
- repair surgically & early
