TEM Flashcards
What is threat management
Manage every day operational problems: Fatigue Weather Non standard phraseology
Types of threats
Internal Or External
Types of external threats
Anticipated Unexpected Latent
Examples of external threats Anticipated Unexpected Latent
Anticipated - thunderstorms Unexpected - aircraft malfunction Latent - design fault, illusions, turn around schedule (latent are always there in the background)
Examples of internal Threats
Lack of experience in a role, Language/cultural issues, Fatigue, Team familiarity
What is error management
Manage their own errors Setting incorrect altitude Not confirming ATC Clearance
What is an Error
I crews actions or in actions that lead to: - a deviation from crew or organisational intentions or expectations; - reduced safety margins; - increase the probability of adverse operational events on the ground or in-flight.
Types of Errors
Handling Procedural Communication
Examples of Errors Handling Procedural Communication
Handling - flight controls, automation Procedural - checklist, briefings, call outs Communication - ATC, ground, crew to crew
What is Undesired Aircraft State Management
Manage an unsafe situation Which will result in either Recovery Or incident/accident
What is an Undesired aircraft state
A position, condition, or altitude of an aircraft that clearly reduces the safety margin and is a result of actions by the crew. It is a safety compromising state the results from ineffective error management
Types of Undesired Aircraft States
Aircraft handling Ground navigation Incorrect aircraft configuration
Example of undesired aircraft States Aircraft handling
vertical, lateral, or speed deviations. Unnecessary wx penetration. unstable approach Firm off centre landing
Example of undesired aircraft States Ground navigation
Runway incursion, Wrong bay, Wrong taxiway, Taxi above speed limits
Example of undesired aircraft States Incorrect Aircraft config
Automation, engine, flight control system or W&B events
