- Care Under Fire (CUF)
- Tactical Field Care (TFC)
- Tactical Evacuation (TACEVAC)
Describe the principals of Care Under Fire (CUF)
- Return fire, take cover
- Direct casualty to:
- remain engaged in fight, if appropriate
- move to cover and self aid
- Form plan based on LOC
- If feasible, roll unconscious casualties onto their side
Describe the principals of Tactical Field Care (TFC)
- No longer under effective fire W/ “bubble” of security*
- Casualties moved to CCP
- Initiate MARCHE+ protocol

What does MARCHE/STOP stand for?
Massive Hemorrhage Airway Respirations Situational awareness Triage all other casualties Ongoing documentation Pass on 9Liner/MIST Cooling prevention/litter placement Circulation (BIFT) an litter placement Hypothermia Head injury Eye trauma Everything else
What does BIFT stand for?
Bleeding control
IV/IO Access
Fluid resuscitation
Tourniquet assessment and/or removal
What does MPHAATD stand for?
Monitor vital signs Pain Head-to-toe Address all wounds/fractures Antibiotics Tactical evacuation/prep Document care
List obvious signs of traumatic bleeding
- Pulsing/continuous heavy flow of blood
- Pooling of blood around casualty
- Blood soaked clothing
- Traumatic amputation
- Signs of shock
What should be considered during the “situational awareness” portion of “STOP”?
- Stop what you’re doing, look around
- Check in with Pl 2IC
- What is the tactical situation
- Do you need to move?
- Other casualties?
Define timeline for “Priority A”
To be at hospital facility (R2 or R3) <90mins
Compromised airway, massive bleed, facial/neck burns
Define timeline for “Priority B”
To be at hospital facility (R2 or R3) within 4 hours
Pulseless limb, significant burns
Define timeline for “Priority C”
To be at hospital facility (R2 or R3) within 24 hours
