Adenosine if regular & monomorphic

50-100 J
Synchronized cardioversion dose for narrow, irregular tachy
120-200 J biphasic
200 J if monomorphic
Synchronized cardioversion dose for wide, regular tachy
100 J
Synchronized cardioversion dose for wide, irregular tachy
Defibrillation dose
Adenosine dose
1st: 6 mg rapid IV push
2nd: 12 mg
Procainamide IV
20-50 mg/min until arrhythmia suppressed, hypotension comes, QRS duration increases >50%, or max dose of 17 mg/kg given.
Procainamide maintenance infusion
1-4 mg/min
Avoid if QT prolonged or CHF
Amiodarone IV dose
1st: 150 mg over 10 min
Repeat if VT comes
Amiodarone maintenance infusion
1 mg/min for 6 hrs
Sotalol IV dose
100 mg (1.5 mg/kg) over 5 min
