First step
BSI
Assess the head
- Inspect for DCAP-BLS
- Palpate for TIC
- Use pen light to check for PERRL
- Inspect for raccoon eyes and battle sign behind the ears
- Inspect the mouth for broken teeth or airway obstructions
- Inspect the nose, mouth, and ears for CSF or blood
Treating eye lacerations
CONSCIOUS
- Position the casualty and remove headgear (sits up)
- Visual acuity testing (how many fingers am I holding up)
- Examine the eyes for COBBLES (ask if they are wearing contacts and write it down)
UNCONSCIOUS
- Position the casualty and remove headgear (elevate the head)
- Examine the eyes for COBBLES
TREAT
- Place gauze on injured eye
- Place fox eye shield
- Tape down with two pieces
- Cover the uninjured eye with gauze
- Tape down with two pieces
Assess the neck
- Inspect for DCAP-BLS
- Palpate C-spine for TIC and step off
- Inspect for JVD
- Inspection for tracheal deviation
Assess the chest
- Inspect for DCAP-BLS and equal rise and fall of the chest
- Ausculate four quadrants
- Palpate for TIC
- Observe for progressive respiratory distress
Assess the abdomen
- Inspect for DCAP-BLS
- Palpate for TRD (if there ISNT an abdominal wound)
Treat an abdominal wound
- Position the casualty (flex the knees and turn the head to the side)
- Expose the wound
- Inspect for BED
- Apply a sterile MOIST dressing. Use IV fluids to moisten
- Wrap the bandage and apply tape to secure
Assess the pelvis
- Inspect for DCAP-BLS
- Apply pelvic binder if needed (should already be there from hypothermia lane)
- Gently check for TIC (if NO PELVIC BINDER)
- Inspect genetalia and perineum for MAP-B
Assess the lower extremities
- Inspect for DCAP-BLS
- Palpate for TIC
- Check for PMS
Assess the upper extremities
- Inspect for DCAP-BLS
- Palpate for TIC
- Check for PMS
Treat an arm fracture
CONSCIOUS
- Have the casualty sit up
- Remove all jewelry
- Expose the site
- Check distal pulse and CRT on the injured arm
- Measure and shape the splint
- Secure with elastic bandage leaving room to recheck pulse and CRT
- Recheck pulse and CRT
- Circumferentially wrap in place
- Apply and sling to broken arm
- Swathe broken arm to uninjured side
Perform ongoing assessment

PDIVE every 5 to 15 minutes
Gather vital signs
(Second to last step)
Using SPO2 and asking “what is the HR, SPO2, RESP, and blood pressure?”
