Systemic complications of LA Flashcards
What are the medical emergencies?
S-P-O-R-T
Decreased CO2 levels leads to
alkalosis
Decreased blood ionized ca levels leads to
neuromuscular irritability as tingling, twitching, paresthesia, muscle spasm and convulsions
What drug prescribed at hyperventilation?
Valium 10mg orally to calm the patient
LA allergens are (predisposing factors)?
Ester L.A agents
Preservative Methyl paraben
Topical Anesthesia
Metabisulfite (V.C Antioxidant)
Allergy to ester-type
Amide type is used
Allergy to amide type
Other Amide type is used
If doubts presist
Dental ttt done via G.A, Histamine blockers, Nitrous oxide and oxygen.
cricothyroidotomy in case of
laryngeal edema
Definitive ttt in Allergy
- Re-assure patient
- Epinephrine … I.M ofEpipen or ( Adults 0.3 ml epi/ /child 15-30 kg = 0.15 ml/ Child<15kg = 0.075 ml )
- Histamine blocker 50 mg Diphenhydramine IM
- Corticosteroids 100 mg Hydrocortisone sodium succinate IM or IV
If Definitive ttt measures fail and respiratory obstruction endangers life
a cricothyrotomy or Tracheostomy performed.
CPR
What is the most cause of local anesthetic toxicity?
Anesthetic is administrated intravascularly
If hypotension persists for extended periods (30 min)
Mild Vasopressor (Ephedrine) is administrated I.M
Adrenaline should be avoided
with poorly hyperthyroid patients and patients taking Thyroid hormone (Thyroxine)
