What are the four systemic fungi that can cause pneumonia and disseminate?
Histoplasmosis, Blastomycosis, Coccidiomycosis, Paracocciomycosis
Where is the endemic location of histoplasmosis?
Mississippi/Ohio River Valleys.
What does histoplasmosis look like histologically?
Macrophage filled with histoplasma (intracellular!). Smaller than RBC.
What animal source is histoplasmosis associated with?
Bird and bat droppings
Under what conditions do histoplasma grow?
As a mold at 20 degrees, yeast at 37 degrees.
What does the presentation of histoplasma look like?
- Asx in most
- Can present w/ pneumonia w/ calcified lesions
- Disseminated: can occur in any organ incl lung/spleen/liver
Where is the endemic location of blastomycosis?
States east of the Mississippi River and Central America.
What does blastomycosis look like histologically?
Broad-base budding, same size as RBCs.
What source is blastomycosis associated with?
Soil and rotten wood
Under what conditions do blastomycosis grow?
As a mold at 20 degrees, yeast at 37 degrees.
What does the presentation of blastomycosis look like?
- Asx - rarely
- Pneumonia, rarely calcified, can be granulomatous
- Usually disseminates - presents w/ weight loss/night sweats, skin/bone involvement (ulcers, granulomatous nodules etC).
What endemic area is coccidiomycosis associated with?
Southwestern US, California. “San Joaquin Valley Fever”
What does coccidiomycosis look like histologically?
Spherule (much larger than RBC) filled with endospores - coccidio crowds.
What source is coccidiomycosis associated with?
Dust – rate increases after earthquakes as spores are thrown into air and inhaled.
Under what conditions does coccidiomycosis grow?
Is a spherule, not a yeast in tissue temperature (37).
