What is a surgical XLA?
A
Raising a mucoperiosteal flap +/- using a drill to section a tooth/remove bone

What is involved in the pre-op assessment?
A
Look in mouth
Check radiograph
- path of withdrawal of roots
- obstacles (intrinsic - tooth or extrinsic - surrounding bone)
- point of elevation
- bone removal
- flap design

What are the 6 principles of flap design?
A
- Surgical access - positioning and size
- Broad base - blood supply and healing
- Avoiding vital structure and healing (mental nerve in 4/5 region)
- Incision over sound bone for healing
- Full thickness flap - mucosal and periosteal layers together - healing
- Intact papillae - healing (poor blood supply = shrinkage and black triangles)
