Surgical options for bilateral TVF paralysis Flashcards
What does universal transverse cordotomy do?
one VF is divided from the vocal process allowing it to retract anteriorly, allowing for a larger posterior gap (improve breathing) and anterior “bunching” (goal of maintain voicing)
What are the cons of universal transverse cordotomy?
can increase risk of aspiration
What does bilateral transverse cordotomy do?
both VFs are divided from the vocal process
What are the pros of bilateral transverse cordotomy?
allow for greater glottal gap while still leaving minimal risk for aspiration and minimal change in voice
What does the complete cordectomy do?
completely removes 1 VF, including vocalis muscle, that is typically used in treating a localized malignancy
completed with laser laryngoscopy
What are the cons of complete cordectomy?
probably wont be able to produce voice
