Technical: Failure of hardware, software and communications.
Data: Poor design, analysis and validation.

Business Environment: Change in environment causing partial or complete obsoleteness e.g. Legislation.

HSBC payment software failed meaning thousands of people in the UK did not receive their wage just before a bank holiday (2015).
RBS had a system failure that meant 600,000 transactions went missing; they were fined £56mil (2015).
NYSE trading was suspened for nearly 4 hours in 2015 after a ‘technical configuration’ problem.

Lack of staff parking, poor terminal signage, delayed flights, staff inadequately trained (baggage), poor communication between desk staff and baggage.
What were the impacts of HT5 delays?
Incoming passengers had 4 hour delays, huge queues, 500 cancelled flights, ‘national embarrassment’, cost BA £25 million
How can we evaluate IT failure?
Functionality: Does the system do what is required?
Usability: Can the intended population use the system?
Utility: Is the system producing the expected benefits?
How can failure be avoided?
User involvement, Education and training, Testing, Management commitment, Evaluation.
Why do systems still fail despite hindsight?
System development highly complex; a balance between the following points is needed: User requirements. Tech requirements. System development method. Organisational characteristics. Resource constraints. Expertise of development team.
