CT scan
-Without contrast to make sure no hemorrhage
-Better than MRI in acute stage
(MRI provides better images and detail, but takes longer than CT)
Thrombolytics (ischemic stroke)
Fibrinolytic:
-Remove thrombi after they have formed
Alteplase (tPA)
MOA: conversion of plasminogen to plasmin
Uses: Acute MI, PE, Ischemic stroke
Route: IV (monitor BP + HR)
AE: increased risk of intracranial bleeding
Antidote: Aminocaproic acid (Amicar)
-Anti-thombolytic
Penumbra Procedure
Goes in with instrument to remove clot (GOAL: within 3 hours)
Consequences of stroke: Location
Depend on the area it effects. especially LEFT vs RIGHT hemisphere
Consequences of stroke: Dysphagia
Trouble Swallowing

-Speech therapy evaluates
Cranial nerves: 5, 7, 9, 10, 12
Consequences of stroke: Motor + sensory deficits

-neglect (no sensory input)
Flaccidity- contralateral side
Spasticity- Within 6 weeks
Consequences of stroke: Visual disturbances
Contralateral field blindness
Hemianopia (loss of half of vision field)
Consequences of stroke: Aphasia

Consequences of stroke: Aphasia: Language Problems
Aphasia: some degree of inability to speak or comprehend
Dysphasia: impairment of speech
Dysarthria: imperfect speech sounds
Word finding problems
Expressive aphasia (comprehend but cannot express)
Receptive aphasia (can communicate but cannot comprehend what is being said to them)

-most memory problems are with names, objects, and words
Consequences of stroke: Behavior problems
- Inc emotional response
- May underestimate own abilities
- Slow reaction time
- Hesitant and cautious
- May be apathetic, confused, disoriented
