How do you dose Dabigatran?

A

If 1 of the following:
Age > 80, concomitant treatment with verapamil
THEN: 110 mg BID

If 2 or more of the following: 
- age 75-80
- CrCl 30-49
-weight /= 3
THEN 110 mg BID

If none of the above, or one of the above,
Then, 150 mg BID
