A
Phenytoin, Carbamazepine, PhenoBarb.

How do you dose Dabigatran?
A
If 1 of the following:
Age > 80, concomitant treatment with verapamil
THEN: 110 mg BID
If 2 or more of the following: - age 75-80 - CrCl 30-49 -weight /= 3 THEN 110 mg BID
If none of the above, or one of the above,
Then, 150 mg BID

If CrCl > 50, THEN 20 mg OD
If CrCl between 15- 50, and two or more: - age>75 - weight / 3 - concomitant treatments THEN 15 mg OD

- CrCl 30-50 ml/min and
60 kg
THEN 5 mg BID - CrCL 15-29 ml/min
