Stroke
A sudden interruption in the blood supply to the brain.
2 main causes:
Ishaemic - blood cot blocking the supply to the brain.
Haemorphagic - leakage in the blood supply to the brain. Weakened blood vessel
Clinical presentation
Dense contralateral hemiplegia Contralateral hemianopia Cortical sensory loss Aphasia, dysphasia Swallowing difficulty - dysphagia Neglect Memory problems Bladder control problems Visuospatial disturbance
Guidelines
NICE
ACSM
NICE guidelines recommend - short, frequent interventions, every day targeted at recovery of mobility.
NICE recommend- positioning, passive movement and active ROM for spasticity and contracture management.
ACSM Guidelines 8-12 reps 1-3 sets 2-3 times a week. Could alter depending on patient fatigue levels.
Outcome measures
Fugi Meyer assessment of motor recovery after stroke - asses sensorimotor impairment
Modified Ashworth scale for tone
Stroke impact scale
6 min walk
Functional independence measures
River mead mobility
Scenario
Patient walking with a pole and with assistance of one, couldn’t turn head to look and walk at the same time.
Eating being able to coordinate hand to mouth movement.
