May have changes in personality

Contralateral motor and sensory loss in face and arms>legs
Contralateral homonyous hemianopia - due to involvement of optic radiation
Cognitive changes
Dominant (L): aphasia
Non-Dominant (R): neglect, apraxia

occipital lobe
How does a stroke in the PCA present
contralateral homonyous hemianopia with macula sparing
Where does the vertrobasilar circulation supply
Cerebellum
brainstem
occipital lobes
How may a stroke present in the vertbrobasilar circulation
Combination of symptoms
- visual: hemianopia, cortical blindness
- Cerebellar: DANISH
- CN lesions
- Hemiplegia
- Uni/bi lateral sensory symptoms
What is lateral medullary syndrome/Wallenberg syndrome?
Occlusion of one vertebral artery or Posterior inferior Circulatory artery
What are the features of Lateral Medullary syndrome
DANVAH Dysphagia Ataxia (ipsilateral) Nystagmus (ipsilateral) Vertigo Anaesthesia - ipsilateral facial numbness and absent corneal reflex and contralateral pain loss Horner's syndrome (ipsilateral)
What is locked in syndrome
Patient is aware and compesmentous
completely paralysed apart from eye movements
What are the causes of locked in syndrome
Ventral pons infarction: basilar artery
Central pontine mylelinolysis - rapid correction of hyponatraemia
