Stroke Flashcards
Location of hypertensive hemmorhage
Basal ganglia - particularly the putamen/external capsule (60-65%)
Thalamic hemorrhage (15-25%)
pontine hemorrhage (10%)
Cerebellar hemorrhage
RARELY can get Lobar ( as per AMIRSYS ER book)
Types of ischemic stroke
large-artery atherosclerosis
cardioembolic
lacunes
Risk of cardioembolism High risk
Mechanical valve mitral stenosis with fib atrial fibrillation left atrial/atrial appendage thrombus Sick sinus syndrome recent MI (
What is hemodynamicall significant narrowing of extra or proximal intracranial vessels?
50-70% or vessel lumen reduced to less than 1.5mm
Where do watershed infarcts occur?
MCA/PCA - posterior parietal region
ACA/MCA - frontal lobes and basal ganglia
Areas affected by hypoxic ischemic injury
Hippocampus (Ammon's horn) Globus Pallidus Amygdala (anaterior choroidal -posterior watershed) Cerebellum Occipital lobes
Definition of lacune
Infarct less than 15mm
Location of lacunar infarcts
Basal ganglia
Internal capsule
Pons
Corona radiata
