Stridor & Croup Flashcards
7 mo girl from Shah Alam, had low grade fever for 2 days, runny nose 2 days and noisy breathing and hoarse voice 1 day. She was well till 2 days ago when she developed symptoms. Was breastfeeding well, alert, active & playful
Had watery nasal discharge
Noisy breathing and develop hoarse voice
Able to lie down flat but noticed to have breathlessness 12hrs before admission.
What is the diagnosis
Laryngotracheobronchitis
What sign can you see in neck radiograph of croup?
Steeple sign
Clinical assessment of croup(wagener)
What is mild croup?
Stridor with excitement/ at rest with no respiratory distress
What is moderate croup
Stridor at rest, w intercostal, subcostal or sternal recession
What is severe croup
Stridor at rest with marked recession, decreased air entry, & altered level of consciousness
Management for mild croup
Oral or parenteral dexamethasone 0.15mg/kg single dose
Management for moderate
Oral or parenteral dexamethasone 0.3 -0.6mg/kg single dose
Mx for severe croup
Nebulised adrenaline & budesonide & parenteral dexamethasone
What if no improvement in severe or deteriorate severe croup
Intubate & ventilate
