What is a stoma?
An artificial opening of the abdomen to divert flow of faeces or urine into an external pouch located outside of the body
Which dosage forms should be avoided in stoma patients and why?
EC/MR capsules are not suitable = insufficient release of active ingredient
Which dosage form should be used in stoma patients?
Forms with quick action (dissolution and absorption): liquids, capsules, and uncoated or soluble tablets
What advice to offer a patient after the use solid-dose forms?
The contents of the stoma bag should be checked for any remnants.
Which medications can cause a diarrhoea effect in stoma patients?
Sorbitol
Magnesium antacids
Iron (only in ileostomy patients)
Which medications can cause a constipation effect in stoma patients?
Opioids
Calcium antacids
Aluminium hydroxide antacids and iron (concern in colostomy patients)
Which medications can cause GI irritation and bleeding in stoma patients?
NSAIDs
What effects do antidiarrhoeal drugs have in stoma patients?
Reduce intestinal motility and decrease water and sodium output
In which patients is loperamide’s effect disrupted?
Patients with a short bowel as loperamide circulates through the enterohepatic circulation
Effects of diuretics and laxatives in stoma patients. How to manage?
Dehydration which can lead to hypokalaemia
Use K-sparing diuretics
Potassium supplements (not usually required)
