Q

Calcific uremic arteriolopathy (calciphylaxis)

A

Pathophysiology

Arteriolar & soft tissue calcification
Local tissue ischemia & necrosis
Risk factors

End-stage renal disease
Hypercalcemia, hyperphosphatemia
Hyperparathyroidism
Oral anticoagulants (eg, warfarin)
Clinical manifestations

Painful nodules & ulcers
Soft tissue calcification on imaging
Diagnosis requires skin biopsy: arteriolar calcification/occlusion, subintimal fibrosis
Management

Analgesia & wound care
Treatment of risk factors (eg, high phosphorus, high calcium)
Optimization of dialysis
