what are the 2 types of status epilepticus? explain each one
A
- convulsive status epilepticus: convulsive seizure which continues for prolonged period [over 5 mins] or when convulsive seizures occur one after the other with no recovery in between. emergency and requires medical attention
- non convulsive status epilepticus: less common and less urgent

what is the initial management of convulsive status epilepticus
A
- position pt to avoid injury
- give oxygen
- maintain blood pressure and correct any hypoglycaemia
- give parenteral thiamine if alcohol abuse suspected
- give pyridoxine [vitamin b6] if there is a deficiency
- give iv lorazepam if seizure lasts longer than 5 mins. repeated once after 10 mins if seizure reoccurs or fails to respond.
- can give iv diazepam but side effects [blood clots]. can use an emulsion formulation to avoid s/e
- im diazepam and suppositories absorption is too slow.

if a patient is experiencing status epilepticus, but there are no facilities for resuscitation, what can be offered to the pt?
what to offer if these options fail?
A
- diazepam can be given rectally OR
- midazolam can be given oromucosal solution and given in the buccal cavity
if this does not work…
- give phenytoin sodium, fosphenytoin sodium or phenobarbital sodium. contact intensive care if seizures continue.

what is the management of non-convulsive status epilepticus?
A
- treat with oral anti-epileptics if urgency is necessary.
