Now I will be conducting a procedure that helps me understand how well your eyes are working with one another. (EOM’s) Grabbing a Transilluminator….

A

Explain to the pt that you would like them to follow the light with their eyes and not moving their head. If they feel pain or double vision, to let me know.

A

EOM’s were Smooth, Full and Unrestricted in both eyes.

I will conducting another test, to see how accurately your eyes work with one another? (Saccades)

You explained that you are about to conduct a test (Saccades), what’s next?

A

There is going to be a gold and silver colored ball on opposites end from one another. What I will get you to do is, with both eyes open, look at the gold ball and then silver ball…..which will be in a difference of time of about 10 seconds.

A

Please start by looking at the silver ball, now look at the gold ball…….10 seconds later….silver ball …..10 sec later……gold ball, now look at the silver ball and now look at the gold ball.

A

The next procedure will me to test your peripheral vision one eye at a time.

You explained what the purpose of the Static CVF…what do you want to next?

A

What you will do is, using your left hand, go ahead and cover your left eye. You will then be looking at my only open eye at all times. Please do not try to look around…I will be displaying some finger in the peripheral part of your vision and do the best you can to explain me how many fingers I am holding up.

Room is full illumination and you completed the test at 1 m in distance (Test: Static CVF).

A

The pt has Full two finger counting in all quadrants.

A

The next test will allow me to assess your eye alignment but before we start do you wear any reading glasses?

You are about to do Cover test and asked about glasses, which the pt replied no…whats next?

A

There is a letter here, I would like you to look at all times (20/40 or greater) and 40 cm away. There is no sign of Strabismus when assessing with the Unilateral Cover Test and No phoria at near.

UCT and ACT done.. next?

A

I will confirm if the pt has any phoria with prism (2 to 4 PD BI or BO). The patient has 2 PD Exophoria

You just neutralized the exophoria…what’s up next?

A

The next procedure will allow me to assess how well your eyes work together..REMEMBER TO REPEAT MORE THAN ONCE……..The pt has break at 5cm and recovery at 7 cm

NPC is done…next?

A

Now I would like to assess the connection between your eyes and your brain…Pupil SIZE in bright for OD and OS is 5 mm and in Dim the pupil size are OD 6mm and 7mm for OS.

You measure the pupil size in bright and dim, now?

A

Now I will be flashing light into your eyes, this will allow me to assess the connection to your brain. Start by doing the Direct, Consensual, Swinging flashlight test and lastly Near pupillary response.

You just completed all 4 pupillary tests. Whats next?

A

State the findings. The pts direct and consensual responses were brisk and reactive. The swinging flash light test was brisk reaction with a (-)APD. The near pupillary response was full and responsive.

Pupils are done and you have stated the findings…What’s next?

A

BLOOD PRESSURE…..Now I will conducting a test that measures your blood pressure.

You just told the pt you would like to test for BP… What’s next?

A

Positioning the arm horizontally or at about the heart level. I would also adjust the patient’s sleeve properly, to ensure the artery is not compressed. The Blood Pressure measurement maybe preformed over loose thing clothing.

You stated the positioning and loose clothing to the proctor. What do you do next?

A

Position the sphygmomanometer on the pts arm, stethoscope earpieces and stethoscope to the diaphragm.

You just placed the BP products onto the pt. Now what?

A

I will applying pressure at a steady rate to 200 to 220 mmHg. I will be reducing the pressure at a slow and steady rate of 2 to 3 mmHg per second.

You have decreased all the way down for BP, now what?

A

The Korotkoff Phase 1 for Systolic reading is at 133 mmHg and the Korotkoff Phase 5 for Diastolic is at 90 mmHg. I will be removing the blood pressure cuff from the arm.

A

Ophthalmic lens evaluation… Starting with PAL’s…..

Distance OU, Distance between Optical Centers, Add power OU and Progressive fitting heights OU.

A
