Subjective
What a person says about himself/herself during while taking the health history
Objective
What you as a health professional observe during the physical examination
Inspection
concentrated watching. It is close, careful watching first of the individual as a whole and then of each body system
Palpation
Applies your sense of touch to assess the following factors, texture, temperature, moisture, organ location and size, swelling, vibration/pulsation, etc.
Percussion
Is tapping the persons skin with short, sharp strokes to assess underlying structures
Auscultation
Listening to the sounds produced by the body, such as the heart and blood vessels

Affect
is a temporary expression of feelings or state of mind

Being aware of one’s own existence, feelings, and thoughts and of the environment. it is the most elementary of mental status functions
Delirium
Is an acute confusional change or loss of consciousness and perceptual disturbance; it may accompany an acute illness, and is usually resolved when the underlying illness is treated
Dementia
is a gradual, progressive process, causing decreased cognitive function even though the person is fully conscious and awake, it is not reversible
Depression
Sad, gloomy, dejected; symptoms may occur with rainy weather, after a holiday, or with an illness; if the situation is temporary, the symptoms fade quickly.
Memory
The ability to lay down and store experiences and perceptions for later recall
Perception
An awareness of objects through the five senses
Flat affect
Lack of emotional response; no expression of feelings
Depersonalization
Loss of identity; feels estranged, perplexed about own identity
Elation
Joy and optimism, overconfidence, increased motor activity, not necessarily pathological
Euphoria
Excessive well-being, unusually cheerful or elated, which is inappropriate considering physical and mental condition+
Anxiety
Worried, uneasy, apprehensive from the anticipation of danger whose source is unknown
Fear
Worried, uneasy, apprehensive; external danger is known and identified
Irritability
Annoyed, easily provoked, impatient
Rage
Furious, loss of control
Ambivalence
The existence of opposing emotions towards an idea, object, person
Lability
Rapid shift in emotions
Inappropriate affect
Affect clearly discordant with content of persons speech
Epidermis
Thin but tough top layer of the skin
Dermis
The inner supportive layer mostly consisting of connective tissue or collagen
Subcutaneous
Adipose tissue, fat cells, Lowest level of the layers of the skin
Apocrine
Produce thick milky secretions and open into the hair follicles. Puberty
Hirsutism
Shaggy or excessive hair
Erthema
Intense redness of the skin from excess blood in the dilated superficial capillaries
Jaundice
A yellowish skin color that indicates rising amounts of bilirubin in the blood
Cyanosis
A blueish mottled color from decreased perfusion; the tissues have high levels of deoxygenated blood
Lesions
are traumatic or pathologic changes in previously normal structures
Clubbing
Occurs wit congenital cyanotic heart disease, lung cancer, and pulmonary diseases. In early clubbing the angle straitens out to 18- degrees and the nail base feels spongy to palpation
