Spot Diagnosis Flashcards
Solar lentigo:
A flat, well-circumscribed patch. Colour varies from skin-coloured, tan to dark brown or black, and size varies from a few millimetres to several centimetres in diameter. Solar lentigines are found on sun-exposed sites, particularly the face or hands.
Ink Spot Lentigo (aka Reticulated Lentigo).
Fewer in number compared to Solar Lentigines
Follows sunburn in very fair skinned individuals
(NB: Lentigo is a pigmented flat or sightly raised lesion with a clearly defined edge. Plural-Lentigines)
Lentigo Simplex
The most common form of Lentigo and may be present at birth or develop in early childhood.
Not associated with sun exposure
Round/Oval shaped macule 3-15mm diameter
Jagged or smooth margin
Symptomless, i.e. not painful or itchy
Distinguished from freckles by their darker colour and scattered distribution. Do not darken or increase in number on sun exposure, as do freckles.
Junctional Naevus
Has groups or nests of naevus cells at the junction of the epidermis and the dermis. A flat mole.
Compound naevus
Has nests of naevus cells at the epidermal-dermal junction as well as within the dermis. A central raised area surrounded by a flat patch.
Atypical melanocytic naevus
A mole with unusual features.
Often shortened to “atypical naevus”.
Can be defined as a mole with at least 3 of the following features.
Size > 5 mm diameter
Ill-defined or blurred borders
Irregular margin
Varying shades of colour (mostly pink, tan, brown, black)
Flat and bumpy components.
Lichenoid Keratosis
Inflammatory reaction arising in a regressing existing solar lentigo or seborrhoeic keratosis.
AKA benign lichenoid keratosis, solitary lichen planus, lichen planus-like keratosis and involuting lichenoid plaque. They are one of the causes of atypical solar lentigo.
Dermoscopy-Uniform clusters of grey dots and, depending on the stage of the lesion, may show signs of an original pre-existing lentigo or seborrhoeic keratosis.
Atrophie Blanche
Pattern often seen in scars on the lower leg

Lentigo simplex is the most common form of lentigo. A single lesion or multiple lesions (lentigines) may be present at birth or develop in early childhood. Is not induced by sun exposure, not associated with any medical diseases or conditions. It is also referred to as simple lentigo and juvenile lentigo.
