SPOT Flashcards
What does SPOT stand for?
Sepsis Prevention and Optimization of Therapy
What is SPOT?
an application that monitors all patient labs and vitals in real time and uses the information to identify patients who are at risk for sepsis.
Sepsis kills how many people in hospitals ever year?
1. 450,000
2. 132,000
3. 75,490
4. 270,000
270,000
True or False
One in every 5 patients admitted to hospitals are septic.
False, one in every 3 patients admitted to the hospital are septic
Name 3 of the 6 factors that SPOT looks at to determine if a patient is at risk for sepsis.
- WBC count
- Sepsis Order Sets
3.Antibiotics ordered - Abnormal vital signs
- Positive Sepsis Screens
- Blood cultures
How much time do you have to document a Severe Sepsis Screen after being notified by Tele tech that you have a SPOT alert?
15 minutes
True or False?
It is ok to back time your Severe Sepsis Screening in Meditech if you are busy doing other things at the time it was due?
False, do not back time your Severe Sepsis Screening.
When do we document a Severe Sepsis Screening on a patient?
- In ER triage
- every 2 hours in ER
- on transfer of care
- With any change in condition of patient
- With a change in mews score > 2
- On Admission
What does TIME stand for?
T - Temperature, higher or lower than normal
I - Infection, may have s/s of infection
M - Mental Decline, confused, sleepy, difficult to arouse
E - Extremely Ill, severe pain, discomfort, shortness of breath
What is Sepsis?
The body’s overwhelming immune response to severe infection and can result in damaging its own tissues and organs causing multi - organ failure and death. (SIRS +Infection)
Name the 4 SIRS
- Temperature > 38 degrees Celsius or < 36 degrees Celsius
- Pulse - > 90 beats/minute
- Respirations - > 20 breaths/minute or PaCO2 < 32
- WBC’s > 12,000 or < 4,000 or > 10% bands
What is Severe Sepsis?
Sepsis (SIRS + Infection) + End Organ Damage
What is Septic Shock?
Severe Sepsis (Sepsis {SIRS + Infection} + End Organ Damage) + Hypotension or Lactate > or = 4.0
Who is at Risk for Developing Sepsis?
- Very Young
- Very Old
- Immunocompromised
- Chronic/End stage Disease (Cancer, Cirrhosis, COPD, CHF, Chronic Renal Failure)
- Wounds or Recent Surgeries
- Indwelling Medical Device
- Increased Length of Stay
- Acute/Critical Illness
True or False
It is very important to Validate all vital signs prior to documenting the Severe Sepsis Screening.
True
