PT receives an outpatient referral for a 15-year-old female soccer athlete with a diagnosis of posterior ankle impingement. Pain increases during pushing off with her toes. Which of the following types of bone defect is most often associated with posterior ankle impingement?
1) Os Trigonum
2) Os naviculare
3) Talar neck osteophyte
4) Morton’s toe
1) os trigonum (defined as an accessory ossicle off the talus) is the most common cause. Pain occurs when the extra bone is pushed against the posterior lateral ankle and the heel during forced hyper-plantar-flexion.
Talar neck osteophyte is more often seen with anterior ankle impingement. Os navicular and Morton’s toe are not associated with posterior ankle impingement syndrome. This type of scenario is often seen in adolescent dancers as well.
