Patterns of Intramedullary T2 Signal Change

Start from top right to bottom left

A
  • Central longitudinally extensive spinal cord lesions
    • Ischemia, transverse myelitis, NMO, dAVF, paraneoplastic
    • Systemic illnesses
      • Neurosarcoid
      • Sjögren syndrome
      • SLE
      • Behçet’s dz
  • Patchy, peripheral, short segment = MS
  • (they didn’t talk about this one in the lecture :/
  • Subacute combined degeneration of cord (B12 or copper deficiency)
  • Cord Ischemia (bottom 4)
    • Anterior spinal artery
    • Anterior spinal artery (not as severe)
    • Sulcal artery (comes of ant spinal artery and alternates levels)
    • Posterior spinal artery
  • Anterior horn pattern = chronic compressive myelopathy, poliomyelitis-like syndrome anterior spinal artery ischemia​
