Spinal Classification Flashcards
McKenzie system emphasizes self-management through
active motion exercises
With repeated movement in a specific direction, pain/paresthesiae move proximally in _____ .
centralization
With repeated movement in a specific direction, pain/paresthesiae move distally or become aggravated in _____.
peripheralization
McKenzie system classifies pts into one of 3 categories:
postural
derangement
dysfunctions
Describe PIVM:
passive intervertebral motion
passively put pt. through osteokinematic motion, flex/ext/rotate, to assess motion
Describe PAIVM:
passive accessory intervertebral motion
PA glides
Sahrman “movement impairment syndrome” primarily based on:
assessing posture, flexibility, strength to classify pts into flex/ext categories, then exercise and posture for intervention
Treatment Based Classification System classifies to identify how best to treat which type of pt?
acute
What are the 4 Treatment Based Classification System categories?
specific exercise (active osteokinematic motion)
stabilization
manipulation
traction
Factors favoring Extension category:
symptoms distal to buttock, peripheralize with lumbar flex, centralize with lumbar ext, preference for ext
Factors favoring Flexion category:
Older than 50yoa, preference for flex, imaging evidence of l-spine stenosis
Factors favoring Lateral shift category:
visible frontal plane deviation of shoulders relative to pelvis, preference for lateral translation of pelvis
Factors favoring Stabilization category:
usually < 40yoa, often above average flexibility, maybe (+) instability testing, PA hyper mobility, aberrant motions (catching)
Signs/Sxs specific to postpartum pts:
Greater general flexibility (postpartum, average SLR ROM (>91°)
Positive posterior pelvic pain provocation (P4), and ASLR (Active SLR) and modified Trendelenburg tests
Pain provocation with palpation of the long dorsal sacroiliac ligament or pubic symphysis
Factors favoring Manipulation category:
Hypomobility of the lumbar spine
No symptoms distal to the knee
Recent onset of symptoms (35° for at least 1 hip)
