Spina bifida Flashcards
Classification of spina bifida?
- Spina bifida occulta
- Spina bifida aperta (cystica)
a) meningocele
b) myelomeningocele
Spina bifida occulta?
(=closed NTDs)
1. covered by skin or
connective tissue
2. without herniation
⭢ spinal cord,
meninges, overlying
skin remain intact
Spina bifida aperta (cystica)?
(open NTDs)
Meninges and/ or neural tissue are uncovered
Meningocele?
Meninges (without neural tissue) herniate trough the vertebral bone defect.
Myelomeningocele?
Meninges and parts of spinal cord herniate trough the vertebral bone defect.
Clinical features of spina bifida occulta?
- often asymptomatic
(without imaging) - lumbar skin dimple
- collection of fat
- patch of hair
Clinical features of spina bifida aperta?
- Herniated part is visible
- motor weakness
- sensory deficits
- bladder and bowel
dysfunction - hydrocephalus
- developmental delays,
cognitive impairment
When and what kind of examinations can we detect spina bifida?
- during pregnancy
- ultrasound examination
- examination of mother’s
blood - amniotic fluid analysis
for alfa-fetoprotein
What can we do if examinations are positive?
- Termination of
pregnancy. - Continuation of the
pregnancy.
Continuation of the pregnancy?
- Cesarean delivery and
antibiotics to prevent
CNS infection. - MRI (or CT scans) to
evaluate bone defects
and/ or neural tissue
lesion. - Surgical treatment
- Long term care may
involve
What included surgical treatment?
- For myelomeningocele
fetal surgery (during
pregnancy) may be
indicated. - For spina bifida aperta
surgery soon after
delivery. - For spina bifida occulta
monitoring and possible
surgery later.
What included long term care?
- Physical therapy
- Rehabilitation programs
- Specific treatment of
neurological disorders
What kind of vitamin plays a vital role in the CNS development?
Folic acid (=B9 vitamin)
What can cause folate deficiency during pregnancy?
- Insufficient folate
supplementation. - Drugs that interfere
folate metabolism. - Diabetes mellitus of
mother. - Chromosomal
aberrations, genetic
factors in fetus.
