What is Space Maintenance
Use of appliances to maintain space following early loss of a primary tooth/teeth and which allow the permanent successor/successors to erupt into proper position in the arch.
What is a Space Maintainer
A fixed or removable appliance placed to maintain space created by premature loss of a tooth or teeth.
What are the rationale for Space Maintenance
• Prevent crowding
• Impaction
• Aesthetic problems
• Malocclusion
What are the factors influencing decision to maintain space
Time elapsed
Congenital absence of permanent teeth
Dental age
Sequence of eruption
Bone covering the unerupted tooth
What are the Indications for Space Maintenance
• Early loss of primary first and second molars
• If there will be more than 6 monthly delay before the permanent successor erupts
• When there is adequate space in the arch
• Early loss of a single primary canine
What are the Contraindications of Space Maintenance
• Spaced arches where space loss will not affect the occlusion.
• Moderate crowding where extraction of permanent teeth is required
What are the indications of Band & Loop Space Maintainer
• Space maintenance in the posterior segments e.g. space maintenance for 1o first molar most frequently before the eruption of the first permanent molar.
• Maintain space of either primary first or second molar after eruption of the first permanent molar
•A pair of band and loop SM can be used if a single 1o molar is lost bilaterally before eruption of the permanent incisors.
What are the advantages and disadvantages of Band & Loop Space Maintainer
Advantages
• Succedaneous teeth erupt freely
• Do not interfere with passive eruption
Disadvantages
• Provides no functional replacement of missing tooth.
• Allows supraeruption of teeth in opposing arch.
• Decalcification under bands
Indication for Distal Shoe Space Maintainer
When a primary second molar is lost before eruption of the permanent first molar.
Contraindications for Distal Shoe Space Maintainer
Patients with subacute bacterial endocarditis
Immunocompromised patients
Blood dyscrasia
Poor oral hygiene
What is the indication for Lingual Arch Space Maintainer
Multiple primary teeth are missing and the permanent incisors have erupted
What is the indication for Nance Arch Space Maintainer
Loss of second primary molar bilaterally in the maxilla.
What is the indication of Transpalatal Arch
Unilateral loss of the 2nd primary molar after eruption of 1st permanent molar
What are the components of Nance Arch
• Consists of bands on the maxillary 6’s
• 0.9mm hard stainless steel wire
• The palatal portion incorporates an acrylic button which contacts the palatal tissues.
