When should pain assessment be performed?
When pt:
- reports new pain
- pain score of 4/10 or above
- persistent ^
- reports unsatisfactory pain relief
- exhibits evidence of distress (non-communicative patient)
Appropriate levels of pain mgt (order them from first to last)
- rectal suppository/sublingual
- IM/subcu/IV
- oral
- oral
- rectal
- IM
At what ages should a FLACC be used?
2 months-7 years
After intervention of pain, when do you need to assess pain again?
Within 1 hour of pain relief

Workflow/Checklist when Discharging Patients
“Before discharging patient , check SSRS status to see if the medication is ready, if it’s ready can discharge patient.”
- Discharge Summary, MC , TCU, memo (if required), medication slip for pharmacy , feedback card (check name and IC is correct)
- Ensuring if medications are in the fridge or cassette to return to patient or pharmacy
- Ensure bedside is clear , if patient have X-ray films or scans, to return back to them
- Then approach patient , remove IV plug if necessary, pass them the documents, medication and films ( to counter check with parents name and IC for the patient is correct )
- Ask for patient’s parents name and NRIC to help with the discharge checklist
- See what time patient leave the ward , write it in notes ( must remember to complete the education and care plan as well as remove IV plug from avatar ) and discharge patient from system.
- If patient is for readmission must remember to ask dr to complete the readmission form and pass to clerk
- Before discharge patient and if it’s a short stay do ask if patient need to claim insurance and make sure patient stay atleast 8 hours
- Make sure clear the documents on clipboard once patient discharged
- Check for bed isolation status after pt left ward. To decide type of cleaning. If isolation, make sure go to EHIMS to pick the bed and choose type of cleaning etc, hpv, terminal cleaning. If hpv need to inform them as well.
