tendinopathies (including tears and ruptures), enthesopathies (inflammation of a tendinous origin from or insertion into bone), cartilage tears, labrum tears, tenosynovitis, capsulitis and non-infective fasciitis
what do the vast majority of soft tissue problems settle with
rest, analgesia and NSAIDS
when can steroid injections be used
for rotator cuff problems and lateral epicondylitis
when can steroid injections not be used
for extensor mechanism problems and around the achilles tendon due to the risk of rupturing the tendon
management of refractory cases which do not respond to conservative management
surgical debridement (removal of disease tissue) or decompression (subacromial decompression)
when can a synovectomy be carried out
in rheumatoid arthritis where there is inflammation of the extensor tendons of the wrist or for inflammation of the tibias posterior to prevent its rupture
major tendon tears require
splintage (e.g casting for achilles tendon rupture), surgical repair (for extensor mechanism rupture) or tendon transfer (tibias posterior and extensor policies longus rupture)
labral tears of the acetabulum or glenoid can be
respected or repaired
