Highly TERATOGENIC!!!
Action
Weak inhibitor of neuronal sodium channels, stabilizes resting membrane potential and reduces neuronal excitability
Use
1st line in all generalised seizures
Pregnancy
MHRA: Valproate medicines (Epilim, Depakote) contraindicated in women of child-bearing ages unless in Pregnancy Prevention Programme and NO other alternative. Contraindicated in pregnant women with bipolar disorder and only used if NO alternative
Consult GP immediately if pregnant / planning

Specialist supervision
7 day prescription validity, 30 day supply
Use highly effective contraception
1. IUD, POP implant, sterilization
2. x2 methods COC + barrier + reg pregnancy testing
Exclude pregnancy before treatment
Fully informed patient info -signed risk acknowledgement form and annual review
Risk minimisation material to support PPP
For patients - patient guide, patient card, risk acknowledgement form
For HCPs - Healthcare professional guide
Actions for pharmacist every time dispense

2. Risk Acknowledgement Form signed and keep copy for pt, specialist and GP
Side effects
- Hepatotoxicity (FATAL)- report persistent vomiting, abdo pain, jaundice, malaise, drowsiness. Discontinue if abnormally prolonged PROTHROMBIN TIME
* Monitor LFTs - Blood dyscrasias e.g. leucopenia, thrombocytopenia. Report infections signs
* MONITOR FBC - Pancreatitis - report abdo pain, nausea, vomiting
Interactions - anticonvulsant effect antagonised

- Mefloquine
- SSRIs
- Antipsychotics
- TCAs / anti-depressants
Interactions - increase risk of hepatotoxicity
