Mechanisms of action-
1 - Saponification —- Fatty Acids —–》fatty acid salts + glycerol
2 - Hypochlorous acid formation —–》 Liberates Chlorine + H+ (neutralizes bact)
3 - Deamination reaction
4 - High pH - reduced bact. Metabolism
A Special Point - Chlorine liberated is consumed within 2 min… So frequent replenation required…
Adv -
Is an antiseptic + Disinfectant + sterilizing solution…
Is a bleaching agent… by the release of NASCENT O2
A potent tissue solvent…
Lubricating action…
Disadvantages-
- Can’t remove the smear layer alone… but does when combined with CHX..
- Can’t dissolve INorganic components.
- Though a bleaching agent, contacts with RBCs ——> Hemolysis —–> Dentin discoloration… Also, difficult to remove from RC walls since it crystallizes…
- Hazardous due to its tissue dissolving property…
Immediate C/F - Pain
Swelling
Ecchymosis
Long term features -
- Parasthesia
- muscle weakness
- scarring
—–> Rx - IM steroids, Abiotics, Analgesics… If needed surgical intervention with wound debridement…
NaOCl n CHX used alternatively.. How?
Main adv - Effervescent action…
FINALLY HYPO should be used before closing!! Because, if CHX used, ——-> reacts with blood n pulp debris —–> Surgical emphysema! ——> tooth pain!!
